Home and Continuing Care Clinical Specialist

The Home and Continuing Care Clinical Specialist will work within a dynamic interdisciplinary team involved in the development and implementation of a national reporting system for home care. 1. Supports the identification of the needs of all key stakeholders for information and education related to the HCRS/CCRS, including the interRAI Resident Assessment Instruments (RAI-HC© or RAI MDS 2.0©); 2. Participates in the planning, development and delivery of a comprehensive range of products and services designed to support health regionsí implementation of the RAI assessment instruments and submission of high quality data to CIHI for pan-Canadian reporting; 3. Participates in the planning, development and delivery of education and client support relating to front line and strategic use of health information for quality improvement and planning; 4. Participates in collaborative partnerships with provincial/territorial ministries of health, health policy organizations, data users, and other key stakeholder groups to ensure the ongoing relevance and quality of the HCRS/CCRS data, outputs, programs and services; 5. Participates in operational process improvement activities in partnership with the home and continuing care program areas; and 6. Promotes CIHI products and services.
Knowledge and Experience
* University degree in a health science discipline. A Masters degree and/or a second degree in adult education would be an asset. Equivalent combination of training and experience may be considered. * A health professional with a current clinical designation. * Five years experience in the health care environment, preferably in a community or residential care setting. * Demonstrated expertise in client/resident assessment and clinical decision-making.
